Requirements for the Issuance of A Philippine Passport
Original Passport
Two (2) 1.77" x 1.37" recent photos
Duly accomplished application form
Fee: RMB 600 for 32 pages
RMB 700 for 64 pages
In case of lost passport, please submit the additional following documents:
Affidavit of Loss
Police Report
Photocopy of lost passport, if any
Copy of work contract/certificate of employment
Copy of residency permit
Fee: RMB 900 for 32 pages
RMB 1,400 for 64 pages
New passport to be issued upon receipt by the consulate of the confirmation from the post that issued the lost passport.
